Care Rite PLLC was established in July 2012, starting with three clinics servicing the Dyer and Lauderdale counties. The three providers at the time were Dr. Pankaj Srivastava(Dr. Sriv), Dr. Angeli Jainand Sandra Fowlkes, FNP. Since then, Care Rite has welcomed Allison Jowers FNP , Jan Sims FNP , Elesa Miller FNP , Hari Salana MDto the team, each who bring a range of skills and services ready to serve the community. Our services include but not limited to; adult medicine, pediatrics (from newborn through adolescence), continuous glucose monitoring and weight loss counseling with treatment.
