Charles River Community Health

Charles River Community Health is an exciting primary care practice providing medical, pharmacy, dental, behavioral health, and vision services to diverse underserved communities in Allston, Brighton, Waltham and surrounding areas. We have 4 sites, including our new Brighton facility where this position is located. We are affiliated with 3 Harvard Medical School teaching hospitals, use an Electronic Health Record, and are working toward achieving Meaningful Use. We have established a Patient Centered Medical Home for our patients. We need an experienced Primary Care Provider to manage a panel of low income, culturally and linguistically diverse patients from newborns through seniors using an integrated, team-based approach to care.
