Our Home Health Aides can offer excellent companion, homemaker, and personal care services. Each of our home health aides will be supervised by a Registered Nurse (RN). In addition to patient's personal care, our home health aides can perform each (but not limited to) of the following activities: Meal Preparation Grocery Shopping Light Housekeeping We are able to make short, long hours or live-in arrangements as suited to your needs. Skilled Nursing Care: At HumanCare, we employ Registered Nurses (RN) who can deliver a broad range of nursing care services. Based on a thorough assessment of the client's health care needs, we develop an individualized care plan that comprises of basic or advanced nursing activities like skin care, round-the-clock medical care in your home after discharge from the hospital following an operation or accident, administering medication, health management, diet and nutrition management, skills Instruction, assistance for medical appointments and use of prescribed equipment such as the Hoyer lift for patient transfer. Our customized care plan can also be focused on a particular diagnosis such as diabetes, dementia and others. For customized nursing care plans, physician orders and efficient care coordination are important and form integral part of excellent nursing care services. Elderly, Adult, Adolescent and Children Care:HumanCare specializes in providing assistance for seniors. Our adult & elderly care service is for people who are not able to live on their own, but do not yet need the level of continuous nursing care that a nursing home offers. This service allows the patient to stay in their own home and receive the proper care they need. Our team including your dedicated home care service professional will co-ordinate with your physicians and take steps to evaluate and deliver to your specific needs at home.
