The Leamington Police Service provides a certified Disclosure of Personal Information letter to individuals requiring what is commonly referred to as a Criminal Record Check. The applicant must be a resident of the Municipality of Leamington. The applicant must complete the Consent to Disclosure of Personal Information form and pay the prescribed fee. The Disclosure of personal Information letter can only be released to the person named in the application. If you require additional Disclosure of Personal Information letters please make inquiries for these at the time of application. Applicants must attend at the Leamington Police Service - 7 Clark Street West, Leamington and submit a completed Consent to Disclosure of Personal Information form and produce two pieces of valid government identification. The primary piece of identification must include a photograph of the applicant (driver's licence or passport) and the secondary piece of identification should be a birth certificate. The prescribed fee is $35.00 for the completion of the Criminal Record Check unless a letter is submitted upon application indicating the application is for an approved volunteer position. The fee for an application for an approved volunteer position is $15.00. Payment must be made at the time of application and fees are payable by cash only. Applications are accepted 24 hours a day. Exact change is required when payment is made at any time other than Monday to Friday, between the hours of 8:00 a.m. and 4:00 p.m., or on holidays. Applicants must provide the addresses they have resided at over the previous 5 years. If an applicant resided in Calgary and/or Edmonton, Alberta during the 5 years immediately prior to the date of application, the applicant must apply to the Calgary Police Service and/or Edmonton Police Service directly as outlined below prior to making an application in Leamington.
