Since 1995 Lyndoch Living has been delivering aged services to people in their own homes across South West Victoria. We are the largest provider of both case management services in South West Victoria. Our suite of community based aged services provide aged care assessment, case management, service coordination and funding to purchase services to assist people to maintain the independent lifestyle of their choice. Services are accessible from our offices in Portland, Warrnambool and Hamilton from experienced staff with local knowledge of the service system who can provide advice, guidance and support for people in their community. Our programs are focussed on addressing the support needs of older people with complex care needs and their carers who are committed to remain living in their community. Our aim is to support people at home to reduce the incidence of early or inappropriate entry into a residential care facility. Our staff work with clients and families/carers to identify the individuals needs and address them using a range of resources available to us or within the community. However should your needs change and you require increased care and support, Lyndoch Living provides some residential places in Warrnambool.
