Northwest Colorado Health
Established in 1964, Northwest Colorado Health (formerly Northwest Colorado Visiting Nurse Association) creates community health and wellness through public-private and dual-county partnerships in Routt and Moffat Counties. Northwest Colorado Health has been self-supporting since 1967. Northwest Colorado Health has grown to operate facilities in Steamboat Springs, Hayden and Craig. Originally, Northwest Colorado Health only provided public health services and home health care. Over the past two decades, the agency successfully added hospice, aging and comprehensive primary care services. In 2005, Northwest Colorado Health acquired the Haven Assisted Living Facility and began operating a Community Health Center (CHC) in Craig. In 2008 the CHC became a Federally Qualified Health Center. In 2010, the Rollingstone Respite House opened in Steamboat Springs providing a site for flexible respite, hospice, bereavement and adult day services. For nearly 50 years Northwest Colorado Health has embraced continuous improvement and strategic alliances to meet developing community needs. Today, the organization provides health programming across the region to people of all ages, income levels and cultures.
