Pathways

Pathways, a Bay Area not-for-profit founded in 1977 by a group of Stanford physicians and community members, is moving care forward with its home health, hospice, private duty home care, palliative care, and geriatric care management services. Known for its innovative pilot projects, Pathways recently launched a unique in-patient program in partnership with Stanford Healthcare. With offices in Sunnyvale, South San Francisco, and Oakland, Pathways family of services reaches more than 5,000 families annually in five Bay Area counties. Pathways cares for patients wherever they live-at home, in nursing homes, hospitals, and assisted living communities. Affiliated with El Camino Hospital and Sequoia Hospital since 1986, Pathways Home Health has been a HomeCare Elite Top Home Health Agencies award winner for the past four years. Pathways Private Duty has been a Home Care Pulse Best of Home Care Provider of Choice award winner for the past two years.
