QualCare Medical Group is a large multi-specialty group based in Hollywood, Florida, bringing together highly trained, experienced physicians and state-of-the-art diagnostic and treatment facilities throughout Dade and Broward County. We have offices in Hollywood, Pembroke Pines, Weston, Miramar and Plantation, FL. QualCare Medical Group, which was built on the foundation of established primary care medical practices, includes Board Certified Internal Medicine physicians and Family Practice physicians. Alongside, we partnered with a "preferred network" of Board Certified specialty physicians, all of whom work as a team, consulting and collaborating to provide patients with in depth high quality coordinated care. QualCare Medical Group also has an affiliation contract with a large multi-specialty group practice in Hernando County Florida, called Access Health Care, which includes over 85 participating physicians. Access provides a substantial resource for QualCare to create a model of integrated services for the future including: integrated EHR connectivity within the group physicians for scheduling group appointments, risk sharing arrangements on the MA-PD side, and integrated quality initiatives and disease management programs across the medical group with the ability to share "best practices."
