Resolutions Hospice

Resolutions Hospice provides individualized care through an Interdisciplinary team. This team consists of the Registered nurse, Physician, Social worker, Chaplain, Dietician and the Volunteer. Members of the Interdisciplinary team are considered "core" services by Medicare, and must be provided by employees of the hospice. Registered Nurse: Oversees the patient care plan. Makes visits as needed, but no less frequent than every 2 weeks. Directs care to be provided by the other members of the Interdisciplinary team.
