This survey is managed by Sleepio Limited. We are committed to protecting and respecting your privacy. The full survey involves about 80 questions, depending on your responses, and will take about 5-10 minutes to complete. We are committed to: protecting the confidentiality of personal information in any form; complying with best practice in relation to obtaining, recording, holding, using and disclosing information; and conforming to statute law (including the Data Protection Act 1998 and the Human Rights Act 1998). We, or our collaborators at the University of Oxford, may publish aggregate information about our users (for example, that men aged under 30 have the worst sleeping habits in the UK) in the context of providing public health information, conducting academic research and/or marketing and publicity. However, we will not publish individual or personally identifiable responses. If you decide to provide your email address, you are consenting for Sleepio to contact you about opportunities to take part in sleep-related research. You can complete the survey without entering your email address. Please note that if you don't provide your email address, we won't be able to identify your data to remove it from the dataset. If you have provided your email address and you later decide to withdraw your data after completing the survey, please contact us at [email protected]. We will not disclose information about identifiable individuals to third parties except in cases described below under 'Disclosure of your information'.
