Tyler ContinueCARE Hospital opened in May 2004 as a 51-bed Long Term Acute Care Hospital (LTACH) located on the fourth floor of CHRISTUS Mother Frances Hospital - Tyler. The hospital enjoys the best of both worlds by having a relationship with a large 450-bed acute-care hospital yet maintains a small hospital atmosphere. Employees are a dedicated team who work together to deliver the best possible care for our patients.
