Skilled Nursing Services for wound care, infusion therapy, pain management, disease management, patient education, medication management and psychiatric care. Assistance with Activites of Daily Living such as meal preparation, bathing, grooming and transferring. Care Coordination -ATB will be the liaison with all other involved entities providing care to the patient. Ensuring compliance with medications and protocols and that physicians, families and other care providers are fully informed at all times.
