Â Bayhealth, Inc. is managed and operated by graduate-level and master's-prepared, clinicians with wealth of experiences in advanced home health practice to provide comprehensive, holistic, and individualized skilled services to patients in the comfort of their own home. Quality care is our primary concern and the goal we continue to strive for. Â By utilizing case management model, we provide services tailored for individual patients needs. Â Our multidisciplinary approach ensures communication and collaboration among healthcare providers.
