The enjoyment of the highest attainable standard of health is one of the fundamental rights of every human being without distinction of race, religion, political belief, economic or social condition. At Care Plan, Inc we are in the business of advancing the overall health of our patients through a person-centered plan of care that promotes the active involvement of the patient and family in the delivery, assessment, and planning of care. We believe in an outcome based care and strive to increase the quality of health of our patients, regardless of the complexity of the case, disease stage, or prognosis.
