Diamond Home Care has a holistic approach where the ultimate goal of recovery is sought through continuous communication between patients, families, doctors, and our staff of specialists. Our team approach to recovery is as follows: ? Assessment & Goal-Setting: Starting with an in-depth home evaluation by our team leader, we work together to identify the goals of the patient, family, and physician. ? Plan of Care: In collaboration with the physician, we develop a comprehensive, individualized, goal-directed plan of care. ? Treatment: We implement a complete, coordinated plan of care in the comfort of each patient's home. A "Case Manager" is assigned to facilitate the process, and keep the physician updated. We ensure compliance with governmental regulations by documenting and confidentially reporting each patient's progress to the necessary agencies. ? Continued Progress: At the end of our services, there may be continuing goals that require follow-up care at the physician's office, an outpatient clinic, a rehabilitation unit, or through continued home services provided by other sources within the community. We will identify community resources and create a plan to optimize patient safety, well-being, and continued progress.
