Horizon Health Center strives to provide total care to all of its patients. If you do not currently have any insurance, our team will work with you to see if you qualify for any assistance programs. If you don't qualify, you may be eligible to apply for the FQHC Discount program. In addition to the health coverage, there are many programs available to help Hudson County families.
