At Horizon, we see the opportunity to provide hospice service to our Veterans as one of the greatest ways we can show our thanks to them for their sacrifice. Although the criteria for hospice eligibility are the same for Veterans as they are for everyone else, we recognize that Veterans can, and often do, have very specialized needs because of their unique experiences. We do our best to understand and meet those needs as we care for the Veteran and family. Hospice care can be a VA benefit, and we have a great relationship with the Idaho Veteran's Administration, and are one of their select providers of choice for our area of service. As such, we work closely with the VA and your VA case manager to develop and implement the best possible care plan for every Veteran. As a Veteran, you know what it means to be in harm's way on the field of battle. At Horizon, we are committed to making sure you are at peace if the medical battles are behind you and its time for your journey to come to a dignified close. We will be with you, by your side, for as long as you need us there. It's the least we can do, and an honor to do it.
