We participate with many insurance carriers, and are a preferred provider for Delta Dental, Aetna, Ameritas, BC/BS Kansas City, BC/BS Kansas, Cigna, Epoch, Freedom Network, Fortis (Assurant), Guardian, Humana, MetLife, Principal and United Concordia. We want our patients to be informed consumers, so for that reason we would like to make sure that all of our patients know the standard of care at Phye Family Dentistry. Please note that it is the responsibility of the subscriber and/or patient to know your dental insurance policy. As a courtesy we will call the insurance company for a breakdown of your benefits, but that it is never a guarantee of payment. Dental insurance policies can and do vary greatly, and although we will do our very best to give you an estimate of your costs, we cannot know the specifics of each and every one of the different policies. The best way for the subscriber/patient to find out what their policy covers is to call the insurance company directly. They will be able to give you what benefits are covered under your specific policy. You can also request a booklet outlining your specific plan from your employer’s insurance coordinator.
