Sheepshead Nursing and Rehabilitation Center

We are a medical model program. Our staff includes a social worker, registered nurse, certified nursing assistants and recreation activity personnel who follow a care plan that has been customized for each participant. The care plan is developed in conjunction with the personal physician who is kept abreast of the patient's health status. Participants have been shown to improve cognitively and physically when they attend an adult day health care program, which fosters independence, provides social interraction and greater structure to the participants' daily schedule. The following are some of the services we offer in our Adult Day Care program.
