Hunter New England and Central Coast Primary Health Network (HNECC PHN) is committed to developing and testing leading primary care models that will improve the health of our local communities. We know there is a wealth of knowledge and frontline experience across the region. It's our aim to facilitate collaboration between organisations and individuals to test models and share knowledge and learnings. The Collaborative Health Innovation Project is part of our INNOV8 health development initiative to support our commitment to healthy people and healthy communities.
