Walnut Place offers a variety of high-quality programs and services that prove our leadership in the industry, including: Complex Wound Care Program: Immediately upon admission, our board-certified physician and nurses provide wound evaluations and treatments. Our services include lymphedema therapy by certified occupational therapists, combined decongestive therapy, hivamat therapy, patient and caregiver education, nutritional evaluations, IV antibiotics and more, along with Health Navigator Services to ensure proper discharge planning and post discharge follow up. NICHE: Through our participation in the NICHE program - the largest geriatric nursing program available - we are able to offer evidence-based, interdisciplinary approaches that promote positive experiences for the older adults in our care. This leads to greater satisfaction rates for our patients and their families. Health & Wellness Navigation: Our trained Wellness Navigation team partners with residents to anticipate needs and address them-from day-to-day nutrition, fitness and life-enriching activities to preparation and follow-up for hospital care. Trilogy: For post-acute respiratory care, this comprehensive respiratory program is delivered by a respiratory therapist for qualifying diagnoses including chronic respiratory failure (COPD), obesity hypoventilation syndrome, neuromuscular diseases and restrictive thoracic diseases.
